Dental benefits are provided through Delta Dental of Indiana, with two PPO options — a Low Plan and a High Plan. Both cover preventive, basic, and major services, and the High Plan also covers orthodontics for adults and children. To keep your out-of-pocket costs down, use a Delta Dental PPO or Premier dentist; network dentists can’t balance-bill you.

In-network amounts apply when you use a Delta Dental PPO or Premier dentist. With a nonparticipating dentist, the plan pays based on Delta Dental’s allowed fee, and you may be billed the difference.
